1. Valid passport 6 months valid from the date of travel includes old passport
2. Visa application form
3. Two (2) recent passport size photo colored (dark clothes ) with white background.
4. Certificate of employment stating position, tenure of service and gross annual income. With approved and signed leave of absence (for employed) / Business name and or SEC Registration Certificate (for Business owner)
5. Original bank certification with 3 months bank statement and or passbook
6. Latest income Tax return
7. Hotel Confirmation
8. Flight Itinerary
REMARKS:
• Applicants who ARE NOT holders of valid/expired used visas: USA, Canada, Japan , Australia, New Zealand and 2 issued Korean visas might be REQUIRED to appear personally at the embassy. Processing of visa will take minimum of 8 working days upon filing.
• Additional cost of Php 1,500 will be charged for more than 59 days of stay.
