1. Valid passport 6 months valid from the date of travel includes old passport
2. Visa application form
3. Two (2) recent passport size photo colored (dark clothes ) with white background
4. Daily Schedule in Japan.
5. Original Birth Certificate form NSO Issued within 1 year (1st time traveler to Japan)
6. Original Marriage Certificate form NSO Issued within 1 year (1st time traveler to Japan)
7. Certificate of employment for Employees
8. Copy of Certificate of Business Name or SEC Registration if owner of the company.
9. Original Bank Certificate with at least three months bank statement
10. Copy of income tax return
11. Copy of Cruise booking information
NOTE:
If NSO Birth Certificate is late Registration born from 1950 to present should submit a copy of the following documents:
1. Baptismal Certificate
2. Form 137
If NSO Birth certificate is unreadable or No record, please submit the original of:
1. Birth Certificate form the Local Civil Registrar.
VISA FEE: Gratis
PROCESSING FEE: PHP 3,500
PROCESSING TIME: Minimum 7 working days upon filing
